Birth Certificate - Due at Time of Registration
All students new to ICD, Preschool through 8th grade

Parenting Plan - Due at Time of Registration
Required from all divorced or separated parents. Please provide the school a copy of the MOST CURRENT VERSION of your Parenting Plan - the section of your paperwork that deals with physical and legal custody, custody arrangements, transportation arrangements, children's address for mailing purposes, education records access, educational decision making, and financial decision making related to educational expenses. Notes on Physicals & Immunizations:

  • Current Physicals and Immunization Records for required grade levels and all students new to ICD are due April 30, 2023.
  • “Current” is defined as having been performed within the last calendar year
  • If you have an appointment for a physical and immunizations scheduled after April 30th, please upload your child’s most recent physical and/or immunizations, and have your doctor’s office fax (636-265-0609) or email ([email protected]) the nurse to provide confirmation your upcoming appointment is “on the board.”
  • Any physical form used by your doctor’s office is accepted, or you may print a form here: Physical Form
